A Crohn-betegség krónikus gyulladásos kórkép, amely a gastrointestinalis traktus bármely területét érintheti. A betegség patogenezise csak részben ismert, különböz? környezeti és egyéni (például genetikai, epithelialis, immun- és nem immun-) tényez?k együttesen vezetnek a krónikus gyulladásos folyamat elindításához, amelyben
egyaránt jelent?s szerepe van a pro- és antiinflammatorikus citokineknek és a lamina propria T-sejtjei defektív apoptózisának. A proinflammatorikus citokinek közül Crohn-betegségben a TNF-alfa játssza az egyik legfontosabb szerepet. Az utóbbi években a Crohn-betegség patogenezisének pontosabb megismerése számos új, specifikus (köztük TNF-alfa és TNF-alfa-receptorellenes) biológiai szer kifejlesztéséhez vezetett. A szerz?k az anti-TNF-alfa-kezeléssel kapcsolatos eredményeket foglalják össze. Crohn?s disease is a chronic inflammatory disorder which may affect any part of the gastrointestinal tract. Its pathogenesis is only partially understood; various environmental and host (e.g., genetic, epithelial, immune and nonimmune) factors are involved, together initiating a chronic uncontrolled inflammation, which is partly due to an imbalance between pro- and anti-inflammatory cytokines, and a defective apoptosis of lamina propria T cells. Among proinflammatory cytokines, tumour necrosis factor-alfa (TNF-alfa) seems to play a central role in Crohn?s disease. Over the past years, the increasing knowledge on the pathogenesis of Crohn?s disease has led to the development of a number of biological agents targeting specific molecules involved in gut inflammation, including TNF-alfa and its receptors. This paper reviews the rationale for the use of TNF-alfa inhibitors in the treatment of Crohn?s disease. |
